Does a Hair Transplant Leave a Scar?

The short answer is yes — every hair transplant leaves some form of scarring. The key question is: how visible is it, and can you see it with your normal hairstyle?

The type and visibility of scarring depends entirely on the technique used.

FUT (Strip Method) Scarring

The older Follicular Unit Transplantation (FUT) method involves surgically removing a strip of skin from the back of the scalp. This leaves a linear scar that can be 10–20cm long.

FUT Scar Characteristics

  • Linear scar across the back of the head
  • Typically 1–5mm wide
  • Visible if you shave your hair very short (grade 1 or below)
  • Can be hidden with hair at 3cm+ length
  • May stretch over time

FUT is rarely used today at quality UK clinics, but many patients who had the procedure years ago still carry these scars.

FUE Scarring

FUE (Follicular Unit Extraction) extracts individual follicles using a micro-punch tool (0.7–0.9mm). This leaves tiny dot scars at each extraction point.

FUE Scar Characteristics

  • Tiny dot scars (less than 1mm each)
  • Spread across the donor area
  • Virtually invisible — even with short hair (grade 2–3)
  • Only visible if head is shaved completely (grade 0 or below)
  • No linear scar

DHI Scarring

DHI (Direct Hair Implantation) uses the same extraction method as FUE but implants using a Choi implanter pen. The scarring in the donor area is identical to FUE.

DHI Scar Characteristics (Recipient Area)

  • Extremely minimal scarring at implantation sites
  • The Choi pen creates smaller wounds than traditional FUE incisions
  • Faster healing in the recipient area
  • Virtually no visible scarring once fully healed

Can Scars Be Treated?

  • FUE into the scar — transplanting follicles into the scar line to camouflage it
  • Scalp Micropigmentation (SMP) — tattooing the scalp to match hair colour
  • Laser therapy — to reduce scar thickness and redness

Donor Area Management

A skilled surgeon carefully manages the donor area to prevent over-harvesting. Extracting too many follicles too close together can cause visible thinning in the donor area. The one-surgery-per-day approach allows for meticulous donor planning.
