Why Exercise Matters After a Transplant

Exercise increases blood pressure, heart rate, and body temperature — all of which can affect your healing grafts. Knowing when and how to return to exercise is critical for protecting your results.

The First 48 Hours: Complete Rest

For the first two days, avoid all physical activity. This includes:

  • No lifting, pushing, or pulling
  • No bending over (keep your head above your heart)
  • No sexual activity
  • Walk gently around your home only

Resting with your head elevated at 45 degrees helps reduce swelling and protects the grafts.

Days 3–7: Light Movement Only

  • Walking: Gentle walks are fine and encouraged for circulation
  • Light stretching: No positions that put your head below your heart
  • No gym, no running, no swimming

Sweating can irritate the healing scalp, so keep activity minimal.

Week 2: Light Cardio

  • Light jogging or cycling at low intensity
  • Brisk walking
  • Gentle yoga (avoid inversions like downward dog)
  • No heavy weights

Keep heart rate moderate. If you're sweating heavily, you're pushing too hard.

Week 3: Moderate Exercise

  • Moderate gym sessions (light weights, higher reps)
  • Cycling outdoors
  • Swimming is still not recommended (chlorine and bacteria)
  • No contact sports

Week 4: Most Activities Resume

Most patients can return to their normal exercise routine by week 4, including:

  • Weight training (start at 50% of your usual weights)
  • Running and cardio
  • Most sports

Week 6: Full Activity

By week 6, you can resume:

  • Heavy weightlifting
  • Contact sports (rugby, football, boxing)
  • Swimming
  • All activities at full intensity

Key Rules to Remember

  1. Never touch the grafts with hands, helmets, or hats in the first 2 weeks
  2. Avoid sweating for the first 7 days
  3. No swimming for at least 3 weeks
  4. No sun exposure on the scalp for 3 months
  5. Listen to your body — if something feels uncomfortable, stop

What Happens If You Exercise Too Soon?

Exercising too early can cause:

  • Increased swelling and bleeding
  • Graft dislodgement (permanent loss of transplanted hair)
  • Infection
  • Extended healing time
