Hair Transplant Scarring: FUE vs DHI vs FUT Compared
Does a Hair Transplant Leave a Scar?
Yes. Every hair transplant leaves some scarring. The real question is how visible it is with the hairstyle you want to wear, and that depends almost entirely on the technique.
Modern FUE and DHI leave tiny dot scars that are very hard to see once healed. The older FUT strip method leaves a line across the back of the head. That difference is why FUE and DHI are now the standard at UK clinics, and the only techniques we offer.

FUT Scarring: The Older Strip Method
FUT removes a strip of skin from the back of the scalp, then stitches the wound closed. The follicles are cut from that strip and implanted.
- Leaves a permanent linear scar, typically 10–20cm long
- Visible if the back and sides are cut short (grade 1–2)
- Can stretch over time
- Longer, more uncomfortable recovery
FUT is rarely used at UK clinics now. Many patients who had it years ago still carry the scar, which is one of the most common reasons people come to us for corrective work.
FUE Scarring: Tiny Dots, Spread Out
FUE takes individual follicles with a micro-punch, usually 0.7–0.9mm wide. No strip is removed and there are no stitches.
- Each extraction leaves a dot scar under 1mm
- The dots are spread across the donor area, not concentrated in one line
- They fade to skin colour or slightly lighter over 3–6 months
- Most people can wear a grade 1–2 cut without anyone noticing
Sapphire FUE uses sapphire blades to open the recipient sites. The cleaner incisions mean less trauma and quicker healing where the grafts are placed.
DHI Scarring
DHI extracts follicles the same way as FUE, so donor scarring is the same: tiny dots. The difference is in the recipient area. The Choi implanter pen places each graft directly without pre-made channels, which means:
- Smaller wounds where the grafts go
- Less bleeding and redness
- Faster surface healing
What Affects How Visible Scars Are
Surgeon skill and donor planning
The biggest cause of visible FUE scarring is over-harvesting: taking too many grafts from too small an area, which leaves the back looking thin or moth-eaten. A careful surgeon spreads extractions evenly and leaves enough donor hair for the future. This is a common problem at high-volume overseas clinics. See our Turkey vs UK comparison.
Skin type
Some skin heals with more visible marks. Darker skin can show darker spots at extraction sites for a few months, which usually fade within 3–6 months.
Hair length
The shorter the cut, the more you can see. If you plan to keep a grade 0, tell your surgeon at the start so they can plan for it.
Aftercare
Following your aftercare instructions has a direct effect on healing. Don't pick at scabs, keep the donor area clean, and protect your scalp from the sun for 3 months, as UV can darken healing marks. PRP on the day, included in our price, also supports healing.
Can Existing Scars Be Treated?
If you already have a visible scar, from FUT, an accident or other surgery, there are options:
- FUE into the scar: follicles are transplanted into the scar tissue so hair grows through it
- Scalp micropigmentation (SMP): pigment tattooed into the scalp to blend the scar with surrounding stubble
- Laser treatment: can reduce redness and thickness of some scars
Prevention is always better than correction, which is why technique and surgeon choice matter.

Frequently Asked Questions
Does a hair transplant leave scars?
Yes, but with FUE and DHI the scars are tiny dots under 1mm that fade to skin colour within 3–6 months. FUT leaves a visible line across the back of the head, which is why FUE and DHI are now the standard.
Are FUE scars visible with short hair?
Most people can wear a grade 1–2 cut without visible scarring once healed. At grade 0, faint dots may show on close inspection, depending on your skin and how you heal.
Does DHI scar less than FUE?
In the donor area they are the same. In the recipient area, DHI's implanter pen makes slightly smaller wounds, so the front usually heals a little faster.
How long do FUE scars take to fade?
Scabs fall away within 7–10 days. The dot scars keep fading over 3–6 months, and most people find them hard to see after that.
Can hair transplant scars be removed?
Scars can't be fully removed, but they can be hidden. FUE into the scar, scalp micropigmentation and laser treatment can all make an old scar much less visible.
Can I have another transplant later?
Yes, if the donor area was managed well the first time. Over-harvesting limits future options, so ask any clinic how many grafts your donor area can safely give over your lifetime.
